I understand my responsibility

I understand that I will be charged an additional fee of $5.00 if I am more than 15 minutes late picking up my child, and $1 every minute thereafter*. I understand that I will also be charged for any damages caused to the center not related to normal wear and tear. I also understand that if I may cancel a session without charge with at least 24 hours notice. I am responsible for knowing my schedule, if I miss a session, or cancel with less than 24 hours notice I will be charged for the session.

I wish to become a member of Wired Minds Tutoring for $25 a month. I will receive $5 off all one on one or group tutoring sessions during the month I am a member. Also as a member my child will be able to participate in monthly and quarterly contests, as well as earn fish for the fish tank.

(Membership is automatically included for individuals who sign up for 21 sessions using one of the two bulk options under the packages section.)
